Compounding the issue, the passenger seated in the front left was holding the cardboard box containing the penguin on their lap. The sequence of events leading to the crash unfolded when “the cardboard box slipped to the right, landing on the pilot’s cyclic pitch control lever.”

The report elaborated on the immediate consequences: “Consequently, the cyclic pitch control lever was forced into a far-right position. The helicopter then rolled sharply to the right, and the pilot was unable to regain control in time to avert disaster.”
The aircraft impacted the ground, resulting in substantial damage classified as an aviation accident.
Impact occurred while the helicopter was approximately 50 feet above ground level.
Fortunately, the report confirmed that no injuries were sustained by anyone involved, including the penguin.
