Individuals frequently employ smartwatches to track their cardiovascular health, often by monitoring their daily step count or documenting their average daily heart rate. Researchers are now suggesting an innovative metric that merges these two data points through a simple calculation: dividing average daily heart rate by the average daily step count.

This resulting ratio, termed the Daily Heart Rate Per Step (DHRPS), offers valuable insight into heart efficiency, according to findings published in the Journal of the American Heart Association by researchers at Northwestern University’s Feinberg School of Medicine.

DHRPS as an Indicator of Disease Risk

The study revealed a correlation between less efficient heart function, as indicated by DHRPS, and a heightened susceptibility to conditions including Type II diabetes, hypertension, heart failure, stroke, coronary atherosclerosis, and myocardial infarction.

“It’s an inefficiency measurement,” explained Zhanlin Chen, the study’s lead author and medical student at Feinberg. “It evaluates the strain on your heart. And it only requires a bit of basic math,” he noted.

Expert Perspectives on the DHRPS Metric

Experts in the field have recognized the merit of DHRPS as a health indicator. Dr. Peter Aziz, a pediatric cardiologist at the Cleveland Clinic, acknowledged its potential as an improvement over relying solely on daily step counts or average heart rate for assessing cardiovascular fitness.

DHRPS Reflects Overall Cardiac Efficiency

“The heart’s performance relative to the workload is arguably more significant for cardio fitness,” Dr. Aziz stated. “This metric provides a practical method to evaluate that.”

While DHRPS doesn’t directly assess heart rate during exertion, Dr. Aziz clarified that it still delivers a general sense of cardiac efficiency, which research has linked to disease associations.

Study Validity and Methodology

Dr. Aziz further emphasized the study’s credibility due to its scale. Researchers analyzed Fitbit data from almost 7,000 smartwatch wearers in conjunction with their electronic health records.

Illustrative Example of DHRPS Interpretation

Mr. Chen illustrated the metric’s value with a comparison of two hypothetical individuals. Both individuals achieve 10,000 steps daily, yet one presents an average resting heart rate of 80 (within a healthy range), while the other’s resting heart rate averages 120.

Interpreting DHRPS Values

The first individual would exhibit a DHRPS of 0.008, whereas the second would have a DHRPS of 0.012. A higher ratio suggests a stronger indication of cardiac risk.

In the research, the 6,947 participants were categorized into three groups based on their DHRPS ratios. The group with the highest ratios demonstrated a more pronounced correlation with disease compared to the other participants. The DHRPS metric also proved more effective in identifying disease risk than step counts or heart rates analyzed independently, the study concluded.

“We intentionally designed this metric to be accessible and utilize readily available data,” Mr. Chen explained. “Individuals keen on actively managing their health can easily perform this simple calculation to gain valuable insights.”
