The British-made Covid-19 vaccine from Oxford University and AstraZeneca has today been approved for use – hours before millions more people are plunged into Tier 4 lockdown. 

In a boost for the UK as it recorded more than 50,000 cases for the first time yesterday, the Department of Health has said today’s decision will pave the way for rapid rollout of the jab and a ‘route out’ of the coronavirus pandemic.

The UK has ordered 100 million doses of the vaccine – enough to vaccinate 50 million people – with the first jabs starting on Monday. 

Health Secretary Matt Hancock has said that the gap between the first jab and the second jab will be extended from four weeks to 12 weeks to ensure as many people as possible can be vaccinated before Easter. The same rule will be applied to the Pfizer jab already approved in early December.

Mr Hancock said today’s decision means Britain can ‘accelerate the vaccine rollout’ and ‘brings forward the day when we can get our lives back to normal’, adding: We can say now with confidence that we can get out of this by spring’.

He told Sky News: ‘It is going to be a difficult few weeks ahead. We can see the pressures right now on the NHS and it is absolutely critical that people follow the rules and do everything they can to stop the spread, particularly of the new variant of this virus that transmits so much faster.

‘But we also know that there is a route out of this. The vaccine provides that route out. We have all just got to hold our nerve over the weeks to come.’

The announcement came as millions more people face the country’s toughest Tier 4 coronavirus restrictions this week, with Boris Johnson to decide which regions will endure the changes later today. 

Matt Hancock will announce the changes in the Commons later with insiders questioning whether the vaccine news has been released as a precursor for stricter lockdown restrictions.

Pressure has mounted on the Government to act as hospitals across England warned of increasing strains on services due to Covid-19 patient numbers, which have reached their highest levels during the pandemic, while a record daily high of 51,135 further cases were reported on Tuesday, along with 414 deaths. 

The vaccine approval came as:

  • Millions in the south-west, Midlands and Lancashire are expected to be added to Tier 4 restrictions closing all non-essential shops;
  • Secondary schools are expected to shut until January 11 at the earliest – but scientists want them closed until February. Boris Johnson will make the final decision today;
  • London ICUs have asked major hospitals in Yorkshire if they will agree to take some patients as wards hospital admissions exceed peak of first wave;
  • 51,135 further cases and 414 deaths were reported on Tuesday;

HOW DOES THE OXFORD VACCINE WORK?

The vaccine – called ChAdOx1 nCoV-19 – uses a harmless, weakened version of a common virus which causes a cold in chimpanzees.

Researchers have already used this technology to produce vaccines against a number of pathogens including flu and Zika.

The virus is genetically modified so that it is impossible for it to reproduce  in humans and cause infection.

Scientists have transferred the genetic instructions for coronavirus’s specific “spike protein” – which it needs to invade cells – to the vaccine.

When the vaccine enters cells inside the body, it uses this genetic code to force the body’s own cells to produce the surface spike protein of the coronavirus.

This induces an immune response because it makes those cells look like the virus, which effectively works as a training aid for the immune system to learn how to fight the virus if the real thing gets into the body.

In clinical trials Oxford’s vaccine was 62 per cent effective at preventing a coronavirus diagnosis if given as two doses, and 90 per cent when one half dose was given followed by a further full dose.

The difference, experts say, may be because a smaller dose the first time allows the body to create a better picture of what the virus will look like before it attempts to launch a full-scale attack, as with a full dose, giving quality first and then quantity later.

Tuesday’s infection tally of 51,135 is the highest toll officially recorded by the Department of Health in a single 24-hour period and it marks a sharp 44 per cent rise on last Tuesday’s figure of 36,804. 

The Oxford/AstraZeneca jab, which has been described as a ‘game changer’, was given the green light by the Medicines and Healthcare products Regulatory Agency (MHRA). The UK has ordered 100 million doses of the vaccine – enough to vaccinate 50 million people.

A Department of Health and Social Care spokesman said: ‘The Government has today accepted the recommendation from the Medicines and Healthcare products Regulatory Agency (MHRA) to authorise Oxford University/AstraZeneca’s Covid-19 vaccine for use.

‘This follows rigorous clinical trials and a thorough analysis of the data by experts at the MHRA, which has concluded that the vaccine has met its strict standards of safety, quality and effectiveness.’  

AstraZeneca said it aimed to supply millions of doses in the first quarter of next year as part of an agreement with the Government to supply up to 100 million doses.

Its chief executive Pascal Soriot said: ‘Today is an important day for millions of people in the UK who will get access to this new vaccine. It has been shown to be effective, well-tolerated, simple to administer and is supplied by AstraZeneca at no profit.’

In a statement, Health Secretary Matt Hancock said: ‘This is a moment to celebrate British innovation – not only are we responsible for discovering the first treatment to reduce mortality for Covid-19, this vaccine will be made available to some of the poorest regions of the world at a low cost, helping protect countless people from this awful disease.

‘It is a tribute to the incredible UK scientists at Oxford University and AstraZeneca whose breakthrough will help to save lives around the world. I want to thank every single person who has been part of this British success story. While it is a time to be hopeful, it is so vital everyone continues to play their part to drive down infections.’

And Professor Andrew Pollard, director of the Oxford Vaccine Group and chief investigator of the Oxford trial, said: ‘The regulator’s assessment that this is a safe and effective vaccine is a landmark moment, and an endorsement of the huge effort from a devoted international team of researchers and our dedicated trial participants.

‘Though this is just the beginning, we will start to get ahead of the pandemic, protect health and economies when the vulnerable are vaccinated everywhere, as many as possible as soon possible.’

Data published in The Lancet medical journal in early December showed the vaccine was 62% effective in preventing Covid-19 among a group of 4,440 people given two standard doses of the vaccine when compared with 4,455 people given a placebo drug.

Of 1,367 people given a half first dose of the vaccine followed by a full second dose, there was 90% protection against Covid-19 when compared with a control group of 1,374 people.

The overall Lancet data, which was peer-reviewed, set out full results from clinical trials of more than 20,000 people.

Among the people given the placebo drug, 10 were admitted to hospital with coronavirus, including two with severe Covid which resulted in one death. But among those receiving the vaccine, there were no hospital admissions or severe cases.

The half dose followed by a full dose regime came about as a result of an accidental dosing error.

However, the MHRA was made aware of what happened and clinical trials for the vaccine were allowed to continue.

HOW DO THE OXFORD, MODERNA AND PFIZER/BIONTECH VACCINES COMPARE? 

Moderna and Pfizer/BioNTech have both released interim results of the final stage clinical trials of their vaccines, with both suggesting they are extremely effective. 

Oxford University has published the findings from its second phase, which show the jab provokes an immune response and is safe to use – it is not yet clear how well it protects against coronavirus in the real world.

Here’s how they compare: 

PFIZER (US) & BIONTECH (DE)

mRNA vaccine – Genetic material from coronavirus is injected to trick immune system into making ‘spike’ proteins and learning how to attack them.

Moderna confirmed it will charge countries placing smaller orders, such as the UK’s seven million doses, between £24 and £28 per dose. US has secured 100million doses for $1.525billion (£1.16bn), suggesting it will cost $15.25 (£11.57) per dose.

The US will pay $1.95bn (£1.48bn) for the first 100m doses, a cost of $19.50 (£14.80) per dose.

Expected to cost £2.23 per dose. The UK’s full 100m dose supply could amount to just £223million.

UK has ordered five million doses which will become available from March 2021. Moderna will produce 20m doses this year, expected to stay in the US. 

UK has already ordered 40million doses, of which 10million could be available in 2020. First vaccinations expected in December.

What side effects does it cause? 

Moderna said the vaccine is ‘generally safe and well tolerated’. Most side effects were mild or moderate but included pain, fatigue and headache, which were ‘generally’ short-lived. 

Pfizer and BioNTech did not produce a breakdown of side effects but said the Data Monitoring Committee ‘has not reported any serious safety concerns’.

Oxford and AstraZeneca said there are no serious safety concerns about the vaccine. Phase three trial saw three out of 23,745 participants have ‘serious adverse events’ that were ‘possibly’ linked to the vaccine. All three have recovered or are recovering, and remain in the trial.

In an interview with the Sunday Times, AstraZeneca chief executive Pascal Soriot suggested that further data submitted to the regulator showed the vaccine could match the 95% efficacy achieved by the Pfizer/BioNTech and Moderna vaccines.

‘We think we have figured out the winning formula and how to get efficacy that, after two doses, is up there with everybody else,’ he said.

Nightingale hospitals are being quietly taken apart as medics warn there are too few doctors and nurses to keep the make-shift facilities open.

Health bosses have already started stripping London’s of its 4,000 beds, ventilators and even signs directing patients to wards, while those in Birmingham and Sunderland are yet to re-open.

An eye-watering £220million of taxpayers’ money was splashed on seven Nightingales amid panic hospitals could be overwhelmed by an influx of Covid-19 patients during the first wave.

But many stood empty for months after ministers hailed them as a ‘solution’ to the Covid-19 crisis when they were opened to much fan-fare during the first months of the pandemic to buffer overwhelmed hospitals.

Intensive care doctors yesterday accused ministers of ignoring warnings staffing was already ‘wafer thin’ in intensive care before splurging on the extra capacity, with little regard as to how they would be run.

And as beds were wheeled away from the flagship Nightingale in London – opened by Prince Charles – figures revealed that Covid-19 hospitalisations in England have surged past the peak of the first wave amid warnings from health chiefs they are back in the ‘eye’ of the Covid storm. 

NHS England data shows 20,426 beds were occupied by patients who had tested positive for coronavirus as of 8am on Monday, up from 17,700 a week ago and above the 18,974 recorded on April 12.

Nurses and doctors have taken to the airwaves to warn wards are filling up with Covid-19 patients, and call on Britons not to celebrate over new year for fear of spreading the virus. 

It comes amid warnings England could be plunged into ‘Tier 5’ restrictions within days after scientists were said to have advised Boris Johnson to impose tougher measures than those in November’s lockdown – with secondary schools, pubs and non-essential shops shut.

Although it is unclear if the new measures will be called ‘Tier 5’, SAGE is thought to have warned the Prime Minister that they must be tougher than the Tier 4 restrictions currently in place.

A scientist advising the Government’s top scientists warned yesterday that national restrictions must be imposed to prevent a ‘catastrophe’ in January and February, and said the Government should act ‘early’.

It could become clear whether Tier 4 is putting the brakes on the spread of the virus within days, because it can take up to two weeks for someone who has been infected to develop symptoms and then get a Covid-19 test. But the Christmas break may mean it takes longer for the tiers impact to become clear. 

The spread of the virus is being driven by a mutant strain, scientists have said, which is thought to be at least 50 per cent more infectious than other strains. 

On Monday, Calum Semple, professor of outbreak medicine at the University of Liverpool and a member of the Scientific Advisory Group for Emergencies (Sage), described the vaccine as a ‘game changer’ but said it would take until summer to vaccinate enough people for herd immunity – when the virus struggles to circulate.

‘To get the wider community herd immunity from vaccination rather than through natural infection will take probably 70% to 80% of the population to be vaccinated, and that, I’m afraid, is going to take us right into the summer, I expect,’ he said. 

Prime Minister Boris Johnson chaired a meeting of the Government’s Covid-19 Operations committee last night, when the need for changes to the tier system were agreed. Health Secretary Matt Hancock is expected to share the specifics with MPs later today.

Areas that may be moved from Tier 3 to Tier 4 due to increasing case rates include parts of the East Midlands, such as Northamptonshire and Leicestershire, as well as all areas of the West Midlands metropolitan county. 

Hartlepool in north-east England, along with a handful of areas in Lancashire – Blackburn with Darwen, Burnley, Pendle and Ribble Valley – could also be upgraded from Tier 3 to 4.

The Times also reported that ministers are also considering imposing the country’s toughest measures on parts of the south west and Cumbria.

Saffron Cordery, deputy chief executive of NHS Providers, warned ‘options were narrowing’ and that moving areas into the highest tier was necessary.

‘Pressures on the NHS in some parts of the country are rising at an unsustainable rate. Thankfully, trusts in other areas have been helping out. But with the virus spreading fast alongside mounting winter pressures, the options are narrowing.

‘We urgently need to get ahead of the outbreak. The Covid-19 tier review offers an opportunity to do that. It will require difficult decisions, moving millions more people to the highest level.

‘The Government must act with boldness, speed and clarity in curbing the threat of Covid-19.’ 

Figures from NHS England showed there were 21,787 patients in NHS hospitals in England as of 8am on Tuesday, compared with 20,426 on Monday, and 18,974 at the first wave peak on April 12.

Five of the seven NHS regions in England are currently reporting a record number of Covid-19 hospital patients: Eastern England, London, the Midlands, south-east England and south-west England.

The other two regions, north-east and north-west England, remain below peak levels that were set in mid-November.

Meanwhile the number of further lab-confirmed cases recorded in a single day in the UK hit a new record on Tuesday, rising above 50,000 for the first time, to 53,135.

But officials can’t be sure if this is Britain’s worst day of the pandemic so far because mass testing was only introduced in the UK in May, but it has been estimated there may have been as many 100,000 cases a day at the peak in late March and early April.

Public Health England bosses admitted ‘some’ cases announced yesterday will be the result of a reporting delay over Christmas but warned the figure was mostly the ‘reflection of a real increase’. Data shows around 40,000 of Tuesday’s cases come from samples taken in England on or after Boxing Day. 

The Government said a further 414 people had died within 28 days of testing positive for Covid-19 as of Tuesday, bringing the UK total to 71,567. 

Yesterday’s 414 deaths are 40 per cent lower than the 691 recorded on the same day last week. However, it can take infected patients several weeks to fall ill, meaning the daily death tolls will inevitably spike in the next fortnight.     

One senior doctor said some trusts in London and the South East are considering the option of setting up tents outside hospitals – something normally reserved for sudden events such as terror attacks or industrial disasters – to triage patients.

The number of Covid-19 patients in London hospitals is now higher than levels recorded at the peak of the first wave of the virus, with 5,371 as of 8am on Tuesday, according to NHS England.

During the first wave, the number of patients in London peaked at 5,201 on April 9.

Ambulances were seen queueing outside hospitals including the Royal London Hospital in Whitechapel and Queen’s Hospital in Romford, both in east London, and Queen Elizabeth Hospital, Birmingham, on Tuesday, and a junior doctor in the capital said his his hospital was ‘aggressively overstretched’ by Covid-19 patients. 

Dr Susan Hopkins, senior medical adviser for Public Health England, said the ‘unprecedented levels’ of Covid-19 infection across the UK was of ‘extreme concern’.

More than six million people in east and south-east England went into the highest level of restrictions on Saturday, meaning 24 million – 43% of the population – are now affected. 

Ministers are also reportedly mulling over plans to force areas into ‘Tier 5’ measures, with secondary schools forced to close, but it is expected that in the ‘immediate future’ they will expand Tier 4.

The impact of Tier 4 measures will be revealed within days because it can take up to two weeks for someone who has caught the virus to show symptoms and get a test. But it is feared the Christmas period may muddy the waters due to delays in reporting positive results over the festive period.

Scientists guiding the Government through the pandemic are understood to have advised Mr Johnson to impose tougher measures than those rolled out in November’s lockdown. 

Whitehall sources have told The Telegraph that Tier 4 could be expanded across the entire country following today’s review.

Around 24million people living in London, the South East and the East of England are already under the harshest Tier 4 curbs. 

‘I would expect more than half of England to move into Tier 4, but it wouldn’t surprise me if two thirds end up in the top tier,’ a health official told the publication.

‘There is also real concern about the South African variant which seems to be spreading fast. Unfortunately, more action is needed to combat rising cases across the board.’   

Lockdown measures are also in place across the other three home nations, after mainland Scotland entered Level 4 restrictions from Saturday for three weeks, and a similar stay-at-home order is in place in Wales.

Northern Ireland has also entered a new six-week lockdown, and the first-week measures are the toughest yet, with a form of curfew in operation from 8pm, shops closed from that time and all indoor and outdoor gatherings prohibited until 6am. 

Commenting on Tuesday’s cases, Dr Susan Hopkins, the senior medical adviser at Public Health England, said: ‘We are continuing to see unprecedented levels of Covid-19 infection across the UK, which is of extreme concern particularly as our hospitals are at their most vulnerable. While the number of cases reported today [Tuesday] include some from over the festive period, these figures are largely a reflection of a real increase.

‘It is essential, now more than ever, that we continue to work together to stop the spread of the virus, bring the rate of infection down, and protect the most vulnerable and the NHS.

‘A critical part of this is each and every one of us abiding by the restrictions in place however hard it may seem at this time of the year. It is critical that we reduce our contacts especially mixing between households. We must observe the basic measures – wash your hands, wear a mask and keep your distance from others.’

It comes as one of Number 10’s scientific advisers warned yesterday that England must be plunged into a third national shutdown to prevent a ‘catastrophe’ in the New Year.

Professor Andrew Hayward, an epidemiologist at University College London and member of SAGE, warned the country is entering a ‘very dangerous new phase of the pandemic’.

He called for ministers to ‘learn the lessons’ of earlier waves, when the Government was criticised for being too slow to lockdown, and to act early this time.

Calling on the Government to take swift action to curb the spread of the virus, Professor Hayward said this morning: ‘I think we are entering a very dangerous new phase of the pandemic and we’re going to need decisive, early, national action to prevent a catastrophe in January and February.

‘A 50 per cent increase in transmissibility means that the previous levels of restrictions that worked before won’t work now, and so Tier 4 restrictions are likely to be necessary or even higher than that.

‘I think we’re really looking at a situation where we’re moving into near lockdown, but we’ve got to learn the lessons from the first lockdown.’ 

Professor Hayward said the rise in cases was ‘very largely driven’ by the new, more infectious variant of coronavirus, and suggested that allowing pupils to return to schools would mean stricter restrictions in other areas of society.

He said: ‘We’ve had control measures that were previously controlling the old variant are not enough for this variant.

‘And so if we want to control the new variant we are going to need much tighter restrictions.’

Professor Hayward said he thought schools would have to return ‘maybe a little bit later’ but that it would mean ‘we’re going to have to have increased, strict restrictions in other areas of society to pay for that’.

A quarter of England’s hospitals were treating a ‘dangerous’ number of Covid patients in the run up to Christmas, official figures show amid the looming threat of a third national lockdown. 

MailOnline’s analysis of NHS England data reveals at least a fifth of general beds at 37 trusts were filled by Covid patients in the week ending December 22. Top experts have warned of a danger zone when coronavirus patients breach 20 per cent of hospital occupancy and the disease starts to impact non-Covid services and drive up the risk of outbreaks on wards.  

Two trusts were even operating at double that threshold during the most recent week, with coronavirus taking up 45 per cent of beds at the Medway NHS Foundation Trust in Kent and four in 10 beds at North Tees and Hartlepool NHS Foundation Trust. 

A third of beds at four other trusts across Kent and London — East Kent Hospitals University Trust (37 per cent), Maidstone and Tunbridge Wells Trust (35 per cent), Barking, Havering and Redbridge University Hospitals Trust (34 per cent) and Dartford and Gravesham Trust (33 per cent) — were being used for patients with the disease. But analysts tracking the outbreak fear 14 trusts will breach the threshold 33 per cent threshold by New Year’s Eve.

Coronavirus patients need to be kept in isolation and treated with stringent infection control measures, which requires more staff and man hours and can put additional strain on hospitals. It makes keeping Covid occupancy below 20 per cent essential to avoid disrupting other parts of the NHS.

‘We need to be more or less in a similar sort of messages of stay at home unless you really, really have to, so there’s that combined with incentivisation of testing, incentivisation of isolation – those sorts of things that will carry us through the next few months while we get as many people as possible vaccinated.’

Responding to Professor Hayward’s comments, the Prime Minister’s Official Spokesman said: ‘I would point back to what we have done throughout the pandemic where we have taken action based on the latest scientific and medical evidence.

‘You have seen us do that throughout December when we have moved areas into Tier 4 exactly for the reason to reduce the transmission of the virus and to try and reduce the R rate of the virus in the areas where prevalence is high.

‘As I said, we obviously keep measures under constant review and we obviously keep the latest scientific and medical data under constant review as well.’

The Government has not ruled out tougher new ‘Tier 5’ restrictions, which could see schools and universities close, or the prospect of a new national lockdown in January. 

Fears that a third national lockdown could be on the way grew yesterday as Cabinet Office minister Michael Gove failed to dismiss the idea of putting the entire country in Tier 4.

He said: ‘We review which tiers parts of the country should be in on the basis of scientific evidence.

‘The Joint Biosecurity Centre will be making a recommendation to ministers, but I can’t pre-empt that because it obviously has to be a judgment based on the medical situation. The NHS is under pressure and these are difficult months ahead.’

It comes as analysis of official figures by MailOnline found more than 90 per cent of councils in England saw their coronavirus outbreaks grow before Christmas.

The data suggests Cumbria could be the next area to be plunged into Tier Four later today.

Three of the Tier 2 county’s six boroughs seeing their Covid infection rate – the number of new cases per 100,000 people – double in size during the week ending December 22.

Department of Health statistics show Eden, home to around 50,000 people, had a rate of 422.5 during the most recent week data is available for – up from 200.9 in the previous seven-day spell. It stood at 41.3 at the start of the month.

It means the borough, which includes Penrith, recorded more confirmed Covid cases for the size of its population than several councils already placed under Tier Four, including parts of Surrey, Berkshire and Oxfordshire.

Allerdale (163.7) and Copeland (64.5) also saw outbreaks double in size over the same time-frame. However, the latter Cumbrian borough still has England’s lowest coronavirus infection rate.

And Barrow-in-Furness – another part of the county – was one of just 27 areas that recorded fewer cases week-on-week. England’s 288 other boroughs saw their outbreaks stay stable or grow, with 35 authorities seeing infections double over the same duration.

Local health bosses fear the rapid growth in cases across parts of the county, which borders Scotland, is being driven by the same coronavirus mutation that spread rapidly across the Home Counties. 

Boris Johnson promised the tier allocation would be based on ‘common sense’, with the JBC – a Whitehall body that decides the whack-a-mole strategy – using a set of five criteria to decide which areas need the harshest restrictions.

This includes the overall infection rate for each area, the number of cases in the over-60s, and the speed at which the outbreak is growing or shrinking.

Officials also look at the test positivity rate – the number of confirmed infections for every 100 tests taken – and the pressure on local hospitals.
